Premier Membership (Per Patient):

1. 4 visits including 1 physical annually
2. 1 annual physical lab-work
3. One ECG/year

Premier Preferred Membership (Per Patient):

1. 4 visits including 1 physical annually
2. 1 annual physical lab-work
3. One ECG/year
4. All in house cardiac testing: (One of each/year)
• Stress Echocardiogram
• Cardiac Echocardiogram
• Carotid Ultrasound
• Pulse Volume Recording
• Aortic Ultrasound

Executive (Per Patient):

1. 4 visits including 1 physical annually
2. 1 annual physical lab-work
3. One ECG/year
